Arresto Menor

I have a love child and she is 9 years old. I got married 2009 and have one child with my husband. We decided to change the last name of my elder daughter to his last name through adoption. Can you help me how is it being processed? and how much will it cost and how long will it take. Im for Lipa Batangas. thanks
