Is it legal for a company to require an employee to sign a waiver form, intended to relinquish the company's responsibility over an employee in events of accidents, as pre-requisite for an employee to be able to join in the company outing?

Legal. It is Valid exercise of management prerogative as a condition precedent in your employment as well as an established company policy.
