good day! gusto ko lang po sana humingi ng tulong regarding sa anak namin ng boyfriend ko. may anak po kame, 5 years old. sa birth certificate ng anak ko, hindi namin nilagay yung name ng boyfriend ko as his father dahil naka-petition ang family ko that time. i was only 17 when i gave birth. now, i'm already 22 and hindi na ako covered ng petition ng family namin. ask ko lang if ano po ba ang process para ma-carry ng anak ko yung surname ng daddy nya (my boyfriend) kahit na hindi pa kami kasal. ayaw ko pa magpakasal dahil nag-aaral pa ako ngayon at gusto ko na muna maging financially stable kaming dalawa. pero we're living in together... hoping you could help me... thanks!

the father should execute two documents, an acknowledgment that he is the father and the authority to use surname of the father. there are standard forms at the local civil registrar where your child was registered.

here is an example of the requirement from pateros: http://www.pateros.gov.ph/services/SocServices/civil_registry3.asp#18

no atty needed and no need to pass through court. it will be all handled at the civil registrar.
