ma'am/sir, if can please enlighten me kung anong proper procedure para magamit and mailagay ng daughter ko ang family name ng tatay nya. kakasal lang namin and yung daughter ko was born nung ndi pa kami kasal. sa birth certificate nya mother's name lang ang na fill out, walang kahit anong details ng father nya that was around 2009. gusto naming ipaayos para mailagay ung details ng father nya and magamit na nya family name ng father nya. papano po ba? salamat
