Good Day!

What are documents needed for changing my child's FIRST NAME? He was born with a name JUDAS CAIN last year February 2012, and then his father decided to changed his name into NEW NAME which is JARED CEZR. question lang po attorney, what are possible ways or documents we can provide in changing his name? we only have his Birtch Certificate, Baptismal Certificate and NSO Birth Certificate as a documents. Or do we really need to seek for an attornry regarding this matter.

go to the civil registrar and inquire about change of first name. there are resources also on the web
