How to change the surname of my daughter?

1. I have already a birth certificate of her but registered in quezon city with out my signature on the back?
2. How to change her surname if I am in the province now?
3. How to file it in our province civil registrar?

Hoping for your immediate answer. Because she will be having her christening this coming June 27, 2013..
