Good pm,
Would like to ask po sana if ano po ang pwede kong gawin para macorrect yung details ko sa birth certificate issued by NSO?
My date of birth po kasi is June 1984 which i had been using eversine at pati po sa mga documents ko is ganun din po but when i requested a copy from NSO ang nka indicate po is June 1981..
Also, wala po akong last name na nkaindicate sa NSO copy but only the first name and the middle name.
Ang dapat po kasi sana is Shirley Repo Castro
pero based sa NSO - Shirley Castro lang po ang nakalagay.
D nga po tinanggap ng DFA when i tried to apply for a passport.
Looking forward for your immediate help on this matter.Sobrang namomroblema na po ako sa dapat kong gawin.
