Good day po!..Nagresearch na po ko bout sa mga wrong name and discrepancy of name sa birth certificate pro di ko po makuha ung mas magandang gawin at baka kau lang po makatulog samin.
I am concerned with my friend's case, named Mark. Kumuha kc sya ng NSO birth certificate last March 2013 as requirement to get a passport, then he noticed that there are some problems or discrepancy on birth certificate from civil registrar na ginagamit nyang information kesa sa nasa NSO.Please help us to fix or how to correct the following problems:
FROM CIVIL REGISTRAR (GINAGAMIT NYA)-
1. name: REYMARK
2. surname: REGNIM
FROM NSO:
1. name: ROMNICK
2. surname: REGAIN
We hope for your immediate response. Pede po bang affidavit na lang ang i-attach sa ganyan to get passport,or pano po ba dapat gawin pra maayos po yan. SALAMAT PO!
