Illigitimate child birth cert.problem

May we seek your advice regarding my nephew's birth regitration Atty?

My youngest sister have had a Live-in partner (since they were not married) and they have a child. My sister gave birth to our provincial hospital but was unsure if the child was registered at the Local registry.

The couple are now separated and child is being raised by my sister. We heard (since the guy is distant)that my sister's partner is now with other fincee. Since our father was one one shouldering the needs of the child and that the couple are now separated, our family would want to change the child's last name.. Panu po pag naregister na po pala ng Hospital yung bata Attorney, anu po maganda and Legal na move po para hindi na lang po yung apelyedo nung lalake ang gamitin ng bata? Sa ngayun po e kinder pa lang po yung bata Attorney at di pa din napapabinyagan
