Change of Status from Married to Single for passport

Hi! I'd like to ask what are the requirements to show at the DFA if I'll have my name and status changed from married name to maiden name and married to single status.

My husband and I were married in civil ceremony in 1986. After that, I had my named and status changed from my maiden to married name and single to married, showing the original documents given us.

My husband and I separated after 9 years but I did not do anything to change my name and status.

Just recently, I asked for a copy of my marriage contract from NSO and was surprised to find that there's none. All my IDs and passport are already in my married name. How can I have my passport and IDs be changed back to my maiden name and single status? What are the requirements? Please enlighten me.

Have your marriage annulled or nullified and provide copies of the decision to the LCR to make the changes in your status. Once changed, forward the same to the concerned government agencies as well.

Thank you for your reply. But the marriage has no record in the NSO already. It might not be recorded to the local where I was married as well. Can I just provide an affidavit to the other government agencies since I cannot have a marriage annulled or nullified if there was no record of marriage at all in the first place. Moreover, I don't have money for annullment or nulllification.

if this is just to change the maiden name (meaning don't get married again), request for a cenomar and the write to these agencies to correct your records
