Arresto Menor

I just wanna ask po regarding sa marriage certificate ko, when we got married kasi iba yng first name nya sa marriage contract from his ID's and other documents, not sure ung may birth certificate sya kasi i never see anything sine we got married.

Now the problem is that he is already dead pano ko i-dedeclarena im widow sa NSO eh mag kaiba ang name nya. Thanks sana yung can give me advice ...
