Arresto Menor

I would like to ask po sana about the Admission of Paternity.
I gave birth last March 28, 2012 to a baby girl out of wedlock, in a hospital in Trece Mertires in Cavite. My daughter carry my surname but I want to put in the birth certificate the father of my daughter but the registrar of the hospital insisted to me if the baby will carry my surname, they cannot put the father's name. Actually the father of my baby is willingly wants to execute the Admission of Paternity but still the registrar doesn't want to put his name on the BC. And now, the BC already filed in the local civil registry without the name of the father and w/out Admission of Paternity. Last Week I already got the copy of BC from their municipality. My question is, can we still execute the Admission of Paternity? What will I do to have that affidavit attached to the BC.
Thank you.
