Magpapakasal na sana kami ng bf ko last march, pero ng kumuha ako ng CENOMAR, I was married to my ex na hindi ko na nakikita ko more or less 4 years. Kumuha ako ng MC and there is a copy sa NSO. It is my signature and all, but I dont remember filing anything. No Birth Certificate, No parent Consent (which was indicated on the MC, but my mother is not in the country during that time), the place where the marriage took place is nothing that I am aware of.

Please give me an advice on how to remove this record.

you can file a petition in court to have the marriage declared void if you can prove that no ceremony took place. or other ground: http://www.pinoylawyer.org/t4792-grounds-for-annulment-declaration-of-nullity-legal-separation-and-separation-of-property

well, yes, but you file a petition (document) to achieve the objective which annulment of the marriage.
