How to get a Civil Marriage?

First of all I am a Filipino. I am planning to go back in the Philippines on April 2012 for a 50 days vacation. Me and my companion which I call my wife(she leaves in the phils with our daughter) we have a 9 yearl old daughter since then we never been legally married and my daughter is getting bigger and she still using her mother's name. Now to make all legitimate I want to get married with her, put all things in the right way meaning me and her mother married and our daughter using my name. My problem is I dont have any idea where to start and it keeps bothering me that i dont have much time.
Is there someone here who can tell me the step by step procedure how to do this in Philippines.
