"Unidentified" ang nakalagay sa fathers name sa birthcertificate ng anak ko sa kadahilanang hindi kami magkasama noong inayos ang birthcertificate , ngayon nagsasama na ulit kami at gusto namin ayusin na sana ang birthcertificate ng anak namin at ilagay sya as father ngunit apelyido ko parin ang gagamitin. Pwede ba un at paanong steps ang gagawin? Matagal na namin gusto ayusin ito hanggang sa ngayon na kailangan na namin agad para maging beneficiary na din sya ng healthcard ng tatay nya
