change surname from maiden name to father's name

My mother and father was not married. On my birth certificate, my surname is my mothers', but at the back of the certificate my father wrote that he acknowledge me as his daughter.

Now i'm applying for passport and they denied my papers because of my BC, that according to them eventhough my father acknwledged me, there is no statement that he is letting me use his surname.

What will i do to change my surname to my father's name? What are the possible procedures, how much it will cost and how long it might occur?

is your father still alive? if so, ask him to execute an authority to use surname of father (ausf) and file that at the civil registry where your birth certificate was filed.

Good day..AttyLLL...We're the same problem..i already filed a R.A 9255 or AUSF...pero hindi na po daw ako sakop...mayroon pa po bang ibang solusyon?
