MY friends name is John Pala Ortega. On his TOR, Baptismal, Voter's registration the name is under that name and he also have a copy of the birth certificate from the local civil registrars office with the same name but when he received is NSO birth the name there stated is John Mata Pala, which is his mom's middle name and maiden name. What are the options of John to Fix it. Is it the NSO's fault about what happened to his Birth records? coz when his parents filled out the form, everything were correct. How will he fix it?

if his parents are married, look up or inquire the requirements for legitimization. if they are not, ask his father to execute a notarized affidavit of paternity and submit to the civil registrar. or he adopts his name and corrects his other records

his parents are married but when he was lately registered his parents were not married at that time. What he needs to do to correct it. What is his best option? Will you please be more specific sir?

He was late registered and on the certificate it says ‘Legitimated’ (due to subsequent marriage). What does it means? and It's under the Local registration certificate. Under this Local certificate The name stated is John Pala Ortega but on the PSA / NSO copy it says John Mata Pala. Can you please explain what happened. Was the problem all about how it was encoded by NSO or is there other reason?

if the NSO copy already says legitimated, then he can now validly use the name : Given Name + Mother's Maiden Name + Father's last name. That is the effect of the subsequent marriage and annotation of the fact of legitimization

it doesn't say legitimated. it only says late registered. on the NSO copy the name is John Mata Pala on the local copy / birth certificate it's under John Pala Ortega.

This is what you said. If there is no marking that says Legitimated due to subsequent marriage, then they should inquire at the civil registrar about filing a copy of their marriage contract

in that case, you should bring it up with the local civil registrar for them to forward the updated birth certificate with the PSA else you will be required to file a petition in court

same case din po saakin. pero di po nila mai forward sa PSA ung updated dahil po nawawala daw po ung affidavit na pinirmahan ng parents ko that time. ano po dapat gawin? thank you

if they aren't dead yet, maybe you can request them. or ask the civil registrar if they will accept that you will be the one to submit the copy of the marriage contract and you will issue the affidavit
