Good day,

First of all I would like to thank you for creating this forum site. Gusto ko lang pong itanong, papaano ko po mapapalitan or magagamit surname ng father ko na deceased na 14 years ago. Hindi po sila married ng mother ko, and nakalagay sa birth certificate not married/illegitimate. However, naka-sign ang father ko sa birth certificate. There's no other documents attached with it. What are the requirements and process? Thank you po in advance.
