We have this problem. My niece is an illegitimate child of my brother. And she was registered in 2 different places. One is in the town of camotes in cebu province and the other one is in cebu city. She was registered in camotes in 2001 bearing her mother's surname, while in cebu she was registered in 2006 bearing her father's surname. Since at that time she will be entering in the elementary and one of the requirement was a copy of the local civil registrar's birth certificate. Since we haven't found any record on the local civil registrar's ,that is why we late registered her. When my father get a copy of the NSO birth certificate. That's the first time we knew that she was registered in the town of camotes. And now since her records was bearing our surname, we want to correct it as of this early in her life and also since local high schools her requires a copy of the nso birth certificate as a requirement. What should we do to correct this error.

you have to use the first birth certificate. explain to the school the situation and show the certificate.

hi... i have a problem about my birth certificate. mali po ung middlename ng father ko sa nso birth certificate ko. ano po ang pedeng gawin ko para maitama po un.

what is wrong? totally different middle name or just wrong spelling?
