Falsification of birth certificate?

hi i have read one scenario that they have put a date and place of marriage on their child's birth certificate. i actually have the same case. i gave birth to my eldest when i was 18. that time the nurse of the hospital did not ask a copy of our marriage contract and i was the informant of my daughter's BC. we have put a date and place of marriage so my daughter can carry the surname of his father. now, i want to take my daughter to dubai where i am currently working but i'm worried our documents will not match since i am not married (still) and on her BC i already got married to his father. apart of this i also want to correct the document of my child. will i be charged of falsification of documents even at that time i am just 18 and not matured enough to know the consequences of my actions?is the BC valid since we did not present a MC at the time of registration? please advise me on this. thank you so much.
